MEDICAL

PROVISION OF WELFARE ASSISTANCE TO PERSONS WITH DISABILITY (PWDS)

STEPS:

  1. Register your name
  2. Take note of the briefing schedule and attend the briefing
  3. Attend PWD activities
  4. Communicate constantly with CSWD, if necessary, together with the appropriate associations
  5. For PWD ID request: download the application form, attached Medical certificate and Birth certificate and submit to DSSD office.

SOLO PARENT ID

REQUIREMENTS:

  • Application Form of Solo Parent
  • Affidavit of Child Custody
  • Birth Certificate of children (minor only)
  • Certificate of Employment (COE)

-if applicable

  • Barangay Certification stating Solo Parent, No Live-in Partner
  • Death Certificate (If Applicable)

STEPS: Fill up the Application for Solo Parent ID and submit it to DSSD Office


PROVISION OF ASSISTANCE TO INDIVIDUAL/S IN CRISIS SITUATION

Requirements for Burial Assistance:

PROVISION OF RELIEF ASSISTANCE FOR DISASTER VICTIMS

REQUIREMENTS:

  • Certificate that the client is a victim from barangay
  • Certification from Bureau of Fire Protection for Fire Victims
  • Barangay Stock File for Disaster Victims

STEPS:

  1. Submit requirements to the barangay team.
  2. Submit to a validation
  3. Receive the assistance and sign acknowledgement.
  4. Register in the logbook of disaster victim at the barangay.

NEW BUSINESS

-For Encoding

1. Duly FILLED-UP AND SIGNED Application Form
2. DTI for Single Proprietorship ( Original & Photocopy )
SEC for Corporation/Association/Partnership/POC ( Original & Photocopy ) with updated GIS from SEC if the SEC registration is not present.
CDA for Cooperatives ( Original & Photocopy )
3. Notarized Contract of Lease of place of business is rented
Notarized Affidavit of Non-rental (sole prop)/Secretary’s Certificate (corp). If not renting
Proof of ownership if property is owned.
4. Franchised Agreement/Certificate of Dealership/IPO Registration if the business will use other tradename rather than its own.

CIVIL REGISTRAR’S OFFICE

MARRIAGE LICENSE:

  1. CENOMAR (PSA)
  2. Birth Certificate or Baptismal

Age 18 to 20

  • Bring father with Valid ID, if deceased, submit the death certificate and let mother sign consent (with valid ID).

Age 21 to 24

  • Bring parents with valid ID (Death Certificate of deceased parent/s)
  • If parents are deceased, guardian nearest kin (with ID) may sign in behalf

IMPORTANT: Notice will be posted for 10 days, thus Marriage License will be released on the 11th day.

For Foreigner:

  1. Legal Capacity – Original copy from Embassy.
  2. (Please contact your Embassy in the Philippines via email or call for further instruction)
  3. Photocopy of Passport – First page and latest arrival.
  4. Certificate of No Marriage (CENOMAR) from PSA Office.
  5. If divorced, attach the divorce paper. If window/er, attach the death certificate.

For Filipino Spouse Only:

  1. Birth Certificate
  2. Certificate of No Marriage (CENOMAR)

ON-TIME REGISTRATION OF CERTIFICATE OF LIVE BIRTH

For Married Couple :

For Not Married:
If the child carries the family name of the father:

  1. AUSF-Affidavit to use the surname of the father (must be executed by the mother).
  2. Valid ID of the parents
  3. All affidavits must be notarized by the lawyer of your choice.

If the child carries the family name of the mother:

  1. Valid ID of the parent/s
  2. Affidavit at the back of the Certificate of Live Birth (COLB) must be notarized by the lawyer of your choice.

LATE REGISTRATION OF BIRTH

  1. Negative Result – City Civil Registrar’s Office (NGC)
  2. Negative Result – PSA (Latest Copy)
  3. Baptismal
  4. Growth Chart – 2012 up to present year
  5. Marriage Cert of Parents if:
  • Married before giving birth
  • Birth Certificate of brother/sister – 1 only
  1. SSS Form / E4 or E1
  2. Philhealth MDR
  3. Form 137 – School Record
  4. Voter’s Certificate – COMELEC

DEATH CERTIFICATE

Procedures:

  1. Died in the Hospital – Proceed to the medical record for the death certificate.
  2. Death at Home – Please secure barangay certification and proceed to City Health Office Room 4 for the preparation of the death certificate.
  • To the funeral parlor for the embalmer’s signature
  • City Health Room 16 for the signing of doctor’s review and transfer of the cadaver if the deceased is buried or embalmed outside Bacolod City.
  • Permits and Licensing Division, Office of the City Mayor for the signing of the Cadaver Form and/or issuance of Burial Permit.
  • Office of the City Civil Registrar for the registration of the Death Certificate.
